  This application allows you to search through archived patient data downloaded from Care360 and Tebra.

  • Use the Login option to gain access.
  • Once logged in, you can use the Search features to query patient records by Name.
  • Data is read-only and reflects the exported data snapshot.
  • If you have issues or questions, visit the Contact page.
